Section 504

Process & Background Information

 A student with a disability under Section 504 is one who has a physical or other impairment (i.e. Diabetes, asthma, bi-polar disorder, etc.) that substantially limits one or more major life activities. Major life activities are functions such as the caring for ones self, performing tasks (such as walking, seeing, hearing, speaking, breathing, learning and working). A parent/guardian who believes that their student may have a disability under a 504 plan must contact their Principal of the school to arrange for a Student Success Team Meeting to evaluate the student’s needs. The Principal, regular education teacher, and other staff designated on the 504 plan Team are responsible for implementing, reviewing, and modifying the 504 plan.
