Enrollment [Requirements for enrollment, transfer,etc. From Kindergarten to Grade 12]
- Kindergarten

If you live within the San Lorenzo Unified School
District, you enroll your kindergarten student at your neighborhood school. You
must go to the school between 8:00 a.m. and 3:30 p.m. Bring the following
documents with you:
– Proof of birth date (birth certificate, baptismal certificate or passport)
– Immunization record, including Hepatitis B
– Physical Examination Medical Report. The exam must have been between March 1
and the first day of school
– Two proofs of residency
- Grade 1-12

If you live within the San Lorenzo Unified School
District, you enroll your student at your neighborhood school. You must go to
the school between 8:00 a.m. and 3:30 p.m. Bring the following documents with
you:

- The San Lorenzo Unified School District continues to
experience a growth in enrollment in all schools. Due to the increased number
of students enrolling, the district requires verification of residence from
families of all students entering grade six and grade nine. These students
include both students new to the district and those who have just completed the
curriculum at a district elementary or middle school
Acceptable proofs of residence:
– Escrow papers showing the parent's name and property address
– Rental agreement and receipt for deposit showing name and telephone number of
the rental unit manager and parent's name and property address
– Current utility bill (PG&E, telephone, or water), Cable TV invoice or
service requested invoice (within thirty days of enrollment) showing parent's
name and property address
– Social service statement showing the name of the student and name and address
of parents
- Open Enrollment
- To attend a school within the San Lorenzo School
District other than your school of residence, you must make your request in
writing during the Open Enrollment Application period at your resident school.
Open enrollment requests will be approved on a space available basis. In
schools where the number of open enrollment applications exceeds space
available, a lottery will be held to determine the priority for open
enrollment. Families are notified by August regarding student acceptance into
another school.
The Open Enrollment Application period is a two-week period in April of each
year. Call your school of residence to get the exact dates or look in the
Principal's newsletter for the announcement.
- Inter-District Transfer
- If you live outside of the San Lorenzo Unified School
District and want your student to attend a San Lorenzo Unified School District
school, you must go to the District Office of your resident school district to
request an Inter-district Transfer form. The resident school district must
approve the transfer. If the resident school district approves the transfer, it
is mailed to Special Services at the San Lorenzo Unified School District. There
must be a valid reason and space available before Inter-district transfers are
approved.
If you live within the San Lorenzo Unified School District and want your
student to attend a school in another school district, you must complete an
Inter-district Transfer form in the San Lorenzo Unified School District Special
Services Office or by mail. This transfer must first be approved by the San
Lorenzo School District. It will then be mailed to the other school district
for consideration. There must be a valid reason and space available before
Inter-district transfers are approved. If you move: If you are moving to another school district, please
notify your student's current school. The current school will inform you of the
check out process. The new school will write to the old school for the student
records when you register your student at the new school.

- Where Can I Get Free or Low Cost Health Care?
For assistance with finding a doctor or dentist, getting a free physical exam or obtaining low cost health insurance, call the PUBLIC HEALTH CLEARINGHOUSE AT 1-888-604-INFO or (510) 444-2229.
For assistance in finding a free or low cost immunization clinic, call or e-mail your district nurse.
- What Do I do if My Child Needs Glasses?>

If your child needs glasses and you are unable to afford them, contact your District Nurse.
If you have Medi-Cal, your nurse can help you find an eye doctor who will accept Medi-Cal.
Mary Conway RN participates in a partnership with the Sight for Students Program. This program provides free gift certificates for eye exams and glasses once every 12 months.
To be eligible: the child must be 18 years of age or younger.
The child or parent must be a US citizen or documented immigrant with a social security number.
The child should not have any other vision insurance that covers eye exams or glasses.
The family income must be within 200% of the poverty level. This means a family of 4 can earn slightly over $34,000 a year and still participate in this program.
If you think your child is eligible, contact your District Nurse.
If you are not eligible for those programs, your school nurse can make a referral to the LionŐs Club. The LionŐs Club also provides assistance with eye exams and glasses.
Health Tips
|
– A local optometrist has advised us that some San
Lorenzo students are purchasing contact lenses from a flea market. Students are
using these lenses to change eye color and, occasionally, eye shape. This
practice is dangerous and illegal. The lenses are being sold without an eye
exam or a prescription. Eye infections, as well as corneal abrasions and ulcers
can result from improper use of contact lenses. Please discuss the dangers of
this practice with your child
|
– Do the children in your childŐs classroom wash their hands before eating? If not, consider sending your children to school with moist towelettes or liquid hand soap so that they can clean their hands regularly.

|
– Be sure and update your childŐs emergency card regularly. It is the only way the school can contact you if your child is ill or injured.
|
|
|
– Parents try to teach their children to share, but parents also need to teach their child that some things should not be shared. Food and beverages are personal items. Sharing food can spread germs. Also, some children have food allergies and sharing food could be dangerous. Other personal items that should not be shared include hairbrushes, combs, hats and hairclips.
|
  –Is your childŐs backpack too heavy? Weigh your child and then weigh his/her backpack. The backpack should weigh no more than 20% of your childŐs body weight. For a 100-LB child, the backpack should not weigh more than 20 LBS. For a 40-LB child, the backpack should be no heavier than 8 LBS. Heavy backpacks can cause back pain, muscle fatigue and injury. If your child needs all the books they are carrying in the backpack, consider getting a backpack with wheels.

What Health Screenings are provided for my child at School? [California State Law requires that certain health
screenings be offered to all children attending California public schools.]
The screenings are:
- Vision Screening
- – A distance eye exam, using an eye chart (either the tumbling E or Alphabet letters) is given to students in grades K, 3, 6, and 9.
- Hearing Screening
- – A hearing test is given to students in grades K, 2,
5, 8, and 11
- Scoliosis Screening
- – A check for a postural deformity of the spine is
offered to 7th grade girls and 8th grade boys
In addition to these state-required screenings, the San Lorenzo PTA pays for optometrists to come to the schools and give exams to all kindergarten and 3rd grade students. This exam not only checks for distance vision but also screens for refractive errors such as farsightedness and astigmatism, muscle balance and coordination, and color screening for boys. Vision problems that could impact a childŐs ability to read are detected early through this program. We are a very lucky district to have such a dedicated and supportive PTA. In addition to the cost of this extra program, many volunteers are needed to help run this program. If you are able to volunteer for one day to assist with a screening program, it would be greatly appreciated. To volunteer, you may contact your school nurse directly or contact your school PTA Health Chairperson.

Immunization
What Immunizations Does My Kindergarten Student Need For School?
Students entering kindergarten need to have the following immunizations:
- 4 doses of polio ( 3 doses are allowed if at least one of the doses was given after the 4th birthday
- 5 doses of DTP or DtaP ( 4 doses are allowed if at least one dose was given on or after the 4th birthday)
- 2 doses of MMR (1 dose may be a measles only vaccine, both doses must be given on or after the first birthday
- 3 does of hepatitis B vaccine
- 1 dose of Varicella (Chickenpox) vaccine
What Immunizations do Other Grade Levels Need?
Students entering other grade levels need to have the following immunizations:
- 3 doses of Polio with at least one dose given after the 2nd birthday
- 3 doses of DPT, DtaP, DT or TD with one dose given after the 2nd birthday
- One dose of MMR given after the 1st birthday
What Immunizations do Seventh Grade Students Need?
In addition to the immunizations listed above, seventh grade students need a second dose of measlesĐcontaining vaccine and proof of 3 Hepatitis B vaccines.
Are there any other Health Requirements for School Admission?
Children entering Kindergarten and First Grade must show proof of a physical exam. For the 2001-2002 school year, kindergarten physicals must be dated on or after March 1, 2001. First Grade physical exams must be dated on or after March 1, 2000.
Personal Records
Parents/guardians of special education students are given copies of assessments and the Individualized Education Program at each meeting. If you have lost these records or would like additional copies. Please call Rebecca Oliver at the Special Education Office (510) 317-4765 to request records.

Special education is a program for students with
disabilities. In order to be eligible for special education a student must have

Eligibility for special education is based upon an assessment for special
education. State and Federal eligibility criteria are used to determine which
students are eligible for services. Special education is not a tutoring
program. It is a program for students with disabilities.
Do you have old prescription glasses you no longer need?

The District Health Office collects used glasses for the LionŐs Club. Because these items are prescription, they may not be used in this county. However, the LionŐs Club takes them to Third World Countries and uses the best match they have. People who would otherwise not have glasses are very grateful for this program. You can turn your old glasses into your childŐs school and have them sent to the Health Office using Intra-District Mail. Last year, we collected over 500 pairs of glasses.
Referral
If you think your student has a disability, please
make a referral for special education assessment.
If your child is in grades K-12, you may refer your student for special
education assessment by writing a letter to the Principal of your school. In
the letter request an assessment and tell why you think your student has a
disability.
When the Principal receives the letter a meeting will be set up to discuss your
student's needs and describe the procedure for any assessment(s) that might be
given. Following the assessment, a meeting will be held with the family in
attendance to discuss your student's eligibility for special education for
students with disabilities.
Intern
In Special Education we have current agreements with
California State University Sacramento, the University of Santa Clara and the
University of San Francisco for teachers to complete their special education
credential requirements while teaching in San Lorenzo Unified School District.
A credential program for teachers interested in the Mild/Moderate or
Moderate/Severe Special Education Credentials is available through California
State University Sacramento. Classes are offered in the evenings and on
weekends throughout the year at Spectrum Center in Berkeley.

Transportation
If your student is unable to attend the neighborhood
school because of special education needs the District will transport your
student free of charge on Durham Bus Company. The special education teacher
will work with the Special Services office to set up the special education
transportation if it is necessary. There is no charge for this service.
Transcripts
Special Education
To request records for your special education student,
please send a written request to the Special Education Office or come to the
office in person. In the request, please state the records you would like
copied. The office is open from 8:00 until 4:30 p.m. Monday through Friday. You
must have proof that you are the parent or court appointed guardian to receive
records. Transcripts for students 18 years and older cannot be requested or
released to parents/guardians.
Moving ?
If you are moving to another school district, please
notify your student's current school. The current school will inform you of the
check out process. The new school will write for the student records when you
register your student at the new school.
If you are a current student in the San Lorenzo
Unified School District you may request a transcript from your high school
office. There is no charge for current students.
If you graduated from the San Lorenzo USD, and need a transcript, you may request one by
coming to the Special Services Office at the District Office. The office hours
are Monday through Thursday from 8:00 a.m. to 4:30 p.m. Phone: 510.317.4764. There is a $5.00 charge
for each transcript.
During the summer all transcripts are available at the Special Services Office,
Monday through Thursday from 8:00 a.m. to 4:30 p.m.

Mid-Alameda County Special Education Local Plan Area (MAC-SELPA)
San Lorenzo Unified School District is a member of the MAC-SELPA along with Hayward, Castro Valley, and San Leandro Unified School Districts. The MAC-SELPA is responsible for ensuring that all students have full educational opportunity regardless of the district of residence in the MAC-SELPA. Community Advisory Committee (CAC)
The CAC is comprised of residents from each district with the MAC-SELPA whose role through that body is to give input in the planning and operation of special education in the MAC-SELPA.
