Use the checklist below to gather all required documentation for enrollment. Have documents ready BEFORE starting the online enrollment application! Documents may be uploaded directly to the application, emailed to [email protected], or submitted in person at the enrollment office.
Documents Checklist
Parent/Guardian ID or Other Proof of Guardianship
Acceptable documents include:
- Picture ID
- Passport
- Court Document
Student's Proof of Birth
Acceptable documents include:
- Birth Certificate
- Passport
- Baptism Certificate
- Statement by the local registrar or county recorder certifying the date of birth
Complete Immunization Record/Vaccination History
Enrollment cannot be completed until all CA immunization requirements are met.

Proof/Verification of Residency
You must provide TWO (2) proofs of residency to be eligible for enrollment, one from Section A and one from Section B*:
Section A – One current, original document in the Parent/Guardian's name, dated within the last 30 days:
- Telephone/Internet statement
- PG&E statement
- Cable/TV statement
- EBMUD/Water statement
- Waste Management/Garbage statement
- Other statements (government / medical / bank)
Section B – One document from the following list:
- Current property tax bill
- Current mortgage verification
- Deed of Trust
- Current Close of Escrow
- Rental/lease agreement

Please complete these documents digitally, print them out and complete by hand, or stop by the Enrollment Office for a hard copy. They may be uploaded to the application, emailed to [email protected], or submitted in person.

Submit the following documentation if applicable to your child or situation. Examples of these documents are:
- Court Appointed Guardianship
- Custodial
- Protective Order
Transferring from another school/district
If your student has previously attended a public, private, or charter school, please submit the following documentation:
- Most recent report card/progress report (TK-8)
- For high school transfers, submit the most recent transcript
- Withdrawal form from the last school stating the grade level assigned for the current year (if transferring in the middle of the school year)
- IEP or 504 Plan (if applicable)
