What is Forms?
Microsoft Forms is an app that lets San Lorenzo USD educators create surveys, quizzes, and polls to collect feedback and implement class activities.
Access Forms
As a district staff member or student, open a web browser and go to http://portal.office.com to sign in to your district Office 365 account.
